Post-Secondary Application Package Full-Time Application:

The following FOUR forms must be filled in and returned by the appropriate application deadline for your full-time application to be considered complete.

IMPORTANT: Please make sure that you use the “digital signature” option. DO NOT just type your name as your signature.

If you have not already created your own “digital signature”, you will need to download the forms to your computer once you have entered your personal information and then use adobe reader or another application to create, save and add your digital signature to documents before you submit.

If you have already created and saved your “digital signature”, you simply download the form, add your personal information and add your “digital signature” before you submit.
